Transformation Coaching Firm Service & Terms

The terms and conditions below apply to all coaching provided by I.R.I.S.E (I REALIZE I’M STRONG ENOUGH) Transformation Coaching Firm, I.R.I.S.E. INC. LCC (“the Coach”) to any individual or organization ("the Client") and constitute the contract for the service to be provided by the Coach for the Client. The term 'coaching' covers career coaching, weight-loss coaching, mindset coaching, behavioral modification, leadership training and coaching, life coaching, personal coaching, and youth coaching for Clients and where applicable includes mentoring or supervision services provided by the Coach or the I.R.I.S.E management team.

In return for the fees payable by the Client (or by a third party on their behalf), the Coach agrees to provide the service as described and in accordance with the terms and conditions set out and stated. The Client agrees to pay all fees in advance for the services on the terms and conditions set out below (in situations where a third party pays the fees, the third party counts as an agent acting on behalf of the Client).The date of the first paid coaching session once it takes place shall be deemed the start date for the services and begin of contract. Completion of payment is considered agreement of contract for services listed to be rendered. Participation by any individual in the first paid coaching session constitutes acceptance of these terms and conditions listed.

Privacy Policy: Use of Member Portal and Access to Course Data

At I.R.I.S.E., your transformation is our mission. As part of our commitment to your privacy and the effectiveness of our 5 Week Weight-loss Mindset Transformation Program, this section outlines the responsible use and protection of data within our member portal.

Use and Limitations of Course Data

Copyright Policy: All content provided within the member portal, including but not limited to course materials, video recordings, workbooks, and text, is copyrighted property belonging solely to I.R.I.S.E. Weight Loss & Transformation Coaching. Distribution and Use: Members are expressly prohibited from copying, sharing, distributing, or otherwise disseminating any course data and materials without prior written consent from I.R.I.S.E. This extends to all platforms, whether electronic or print.

Non-Disclosure Agreement (NDA)

Signed Agreement: As a prerequisite for program participation, all members are required to sign a Non-Disclosure Agreement (NDA), outlining the terms of confidentiality. The unauthorized sharing or use of course data constitutes a direct breach of the NDA, and violators may be subject to legal action.

Compliance and Daily Program Adherence

Importance of Compliance: Adhering to the program guidelines and actively participating in the courses are vital elements of your success. Compliance is not only ethical but also essential in ensuring the highest standards of accountability and support. Results and Consistency: We cannot stress enough the direct correlation between daily engagement with the program's elements and the achievement of your weight and wellness goals. Neglecting to consistently follow the program minimizes your potential for success and diminishes the return on your investment in your transformation journey.

We take the responsibility of providing a safe, private, and effective environment for your transformation journey seriously. Failure to abide by the guidelines set forth in this section and in your signed NDA may result in immediate termination of your access to the member portal and may also result in legal action.

Coaching Terms and Conditions Format of Sessions:

Face to Face via (online portal zoom or in person), Telephone coaching sessions or another format only as agreed, and safety/confidentiality is maintained for the coaching client.

The Client is responsible for contacting the coach and logging into the virtual appointment via Zoom or in person at the time of the scheduled coaching session.

The Coach is responsible for ensuring availability for all consultations and or appts at the agreed time in the agreed format previously established. Face to Face via Video conference is the preferred form of contact for all coaching sessions.

Length of Each Session:

Session length is dependent on the services being rendered and the client’s coaching contract. Coach will notify client of service on based on the paid coaching plan and contract.

Initial Number of Sessions to be provided: Sessions per week/month are based on the clients pre- paid pre-paid coaching services or monthly subscription plan. All new clients will have two initial Mapping Module sessions covered in the pre-paid cost of the Transformation Pak prior to select coaching sessions day and time. Per Session Fee. In accordance with the Coach's current per session fee as services offered per the Transformation Coaching Firm fee for sessions, or any other such fee as shall be agreed upon per contract prior to the start of services.

Additional services needed or requested must be prepaid and the client’s current contract will be adjusted:

All I.R.I.S.E Transformation Coaching Programs and Individual sessions are prepaid services and due to the type of services we do not offer refunds. Under special circumstances I.R.I.S.E. Transformation Coaching firm will offer an exchange of services or credited sessions to your member account.

Additional Sessions The Coach may agree to provide additional coaching sessions after completion of the initial agreed session(s). These terms and conditions will apply to any additional sessions so provided and the Per Session Fee will remain the same as originally agreed except where the Coach notifies the Client in writing by e-mail of a change to the Fee or to any other term or condition in accordance with the section in these terms and conditions headed “Variation of Terms and Conditions”.

Dates and Times of Sessions: The date and time of the first session and any subsequent sessions will be booked by client in the system and confirmed by email, SMS or. Sessions can only be rescheduled in accordance with the section in these terms and conditions headed “Rescheduling Sessions. ”Payment Terms Fees can be paid online by debit or credit card or by bank transfer using the link provided on the invoice or other payments portals provided i.e. (Zelle, PayPal, After Pay, Sezzle, Stripe, Klarna, Shop Pay or Affirm). Receipts will be sent by e-mail or SMS text unless otherwise requested. Fees for coaching services are paid in advance unless otherwise agreed. Where payment has not been received by the Coach in advance of first scheduled session, the session will be cancelled and rescheduled once payment has been made. I.R.I.S.E (I REALIZE I’M STRONG ENOUGH) Transformation Coaching Firm, LCC has a no refund policy as a way of honoring the client’s commitment to personal development, informed decision-making, and accountability.

Between Sessions The Coach may assign the Client tasks or exercises to complete between coaching sessions. There is no obligation on the Client to complete these items of ‘growth work’, but not doing so may slow the Client's progress in gaining improved quality of life or achieving desired weight-loss, wellbeing and or personal outcomes and goals. Where possible, Clients are requested to submit any information requested by The Coach relating to assignments at least 24 hours before the coaching session. This will allow both the Client and Coach prepare for the session. The Coach may provide feedback on completed assignments.

The Client may contact the team via mobile app, text or e-mail between sessions to seek clarification regarding anything arising from a coaching session or for administrative purposes (e.g. how a Client reschedules a coaching session or make a payment). Additional coaching can also be provided between sessions. There may be an additional charge for this. The Coach will always advise a client in advance if the nature of a client’s contact is likely to incur an additional charge and no such charges will be imposed without the Client's agreement.

General Coaching Session Guidelines

All appointments and sessions are conducted via Zoom. You will receive a specific Zoom link when you confirm your appointment, which is also included in all reminder texts and emails. Due to liability and company privacy policy, no exceptions will be made. Cameras must be on for the duration of all sessions to enhance the effectiveness of your therapeutic treatment. Clients are expected to be in a private space, free from distractions, to protect confidentiality and privacy.

1:1 Coaching Sessions and Assessments

Late/No-Show Policy: Clients are granted a 5-minute grace period after the scheduled start time. Arrivals beyond this period will result in automatic cancellation, and you will be considered a no-show.

Required Preparation for accountability appointment's and sessions:

You must bring the following to your 1:1 appointments:

Your treatment plan goals

Current weight and measurements

Journals (both required)Updated nutritional progress.

Failure to provide this information will necessitate a cancellation and rescheduling of your appointment.

Group Accountability Session Punctuality:

Arrive on time to make the most of your session and respect others' time.

Cameras are mandatory.

Mute Upon Entry: Please mute your microphone upon entering the session to minimize background noise and distractions.

Late Entry: If you arrive late and the session cannot be interrupted, you will wait in the Zoom waiting room until the instructor can pause to allow your entry.

Session Cancellation: If no clients are present 15 minutes after the session’s scheduled start time, the class will be automatically canceled.

Commitment and Accountability

Your adherence to this policy is critical for ensuring a high-quality experience for all. I.R.I.S.E. is committed to providing an environment where clients can focus fully on their therapeutic weight loss journey, free from the setbacks caused by disruptions and lack of preparation.

We appreciate your understanding and cooperation.

Rescheduling Sessions If a client needs to reschedule a coaching session, they should provide at least 24 hours’ notice. No refunds will be given to Clients for unused coaching sessions that are cancelled.

In exceptional circumstances the Coach may need to reschedule a coaching session. In those instances, she will also give the Client immediate to 24 hours’ notice where practical and possible.

Confidentiality Personal information or business information supplied by Clients in coaching sessions will be treated as confidential and will not be shared. It will not be disclosed to a third party without the Client's prior permission, saved where required by law or where action might be necessary to prevent harm to the Client or someone else.

Early termination In exceptional circumstances, such as inappropriate behavior by the Client, actual or potential conflict of interest, or other reasons, the Coach can decide to terminate the service to the Client early or refuse or be unable to provide further coaching sessions to the Client. In such a circumstance the Client will be given reasonable notice of termination by the Coach where practicable and other services will be negotiated. No Cash refunds will be given. Client will be compensated for payment with gift card or modified services. Responsibilities The Coach will seek to enable the Client to improve his/her self-esteem, relationship with food, weight – loss, mindset, quality of life, and to achieve the desired outcome(s). Remarkable results can be achieved where Clients follow a clear plan in a committed way. However, the Client has sole responsibility for taking important decisions in their life. The Coach has no liability for any loss incurred by any Client, whether financial or otherwise, following commencement of coaching sessions, or for any perceived failure by the Client, whether justified or otherwise, to achieve a material improvement in quality of esteem, life, business or to achieve their desired outcomes or goals.

Variation of Terms and Conditions Where an Initial Number of Sessions is agreed, any changes to these terms and conditions intended to take effect prior to the conclusion of those Initial Number of Sessions will only have effect if agreed by both the Coach and the Client and confirmed by the Coach in writing by email. In other cases, the Coach may change any of these terms or conditions including the Per Session Fee by giving the Client two weeks' notice (when possible) in writing by e-mail of the change(s). If following receipt of such notification of change, the Client no longer wishes to proceed with further coaching sessions, s/he may withdraw from the service immediately by giving notice in writing by email. The Coach will determine if the Client is entitled to a refund and if so, how much based on when the sessions are terminated. Such notice will be effective on receipt by the Coach. Governing Law This contract is governed by the law of Arizona whose courts have exclusive jurisdiction in relation to any dispute, disagreement, proceedings or legal claim of any nature relating to the service provided in the contract.
